CLEAR copy of Passport (the Employee's and that of any
Spouse/Children).
Copies of I-94 cards (only if in the U.S. earlier).
Copies of I-20 forms and Employment Authorization Cards
(only if employee was on a student visa in the U.S.).
Copies of Degree Certificates/Diplomas, and any U.S.
Equivalency Evaluation of Foreign Education (if
available).
Copies of certificates of membership in any professional
organizations/societies or professional licenses.
Copies of college/university transcripts, grade sheets,
or mark sheets.
Copies of experience letters (if readily available).
Copy of resume/ curriculum vitae.
Personal information about Employee/Family members:
names, dates of birth, places of birth; address in home
country, social security numbers (if any). Please fill
the data in the format shown in the pages below.
Copies of All Prior H-1 Approval Notices and Petitions
(only if earlier in the U.S. on H-1 visa for another
employer).
Copies of All Prior H-4 approval notices for any Family
members (if applicable),
